OBTAIN A REPEAT PRESCRIPTION?
Telephone requests are not accepted. Prescriptions
may be collected after 48 hours, during surgery opening times, by a responsible
adult, never children under 16. We will post it to you if you leave a stamped
addressed envelope with your request but please allow more time. Even if you
feel well, arrange to see your doctor by the review date if one is written on
your presciption.

OBTAIN SICK NOTES?
Self-Certification is the correct
procedure to use if you are ill and unable to work for up to seven days. The
form is available from your employer or from the post office. After this, if
you are still too ill to work, you will need to see a doctor to get a DWP certificate. |
